Do you know a special someone who has made such a difference or gone that extra mile, that you feel they should be recognised for their efforts? Are you part of a group that has worked to improve the lives of others or made a significant contribution to the community?

Then you should nominate them for a Stars Of The Community award. By nominating someone for Bolton At Home's Stars Of The Community awards, you could help them win £100 to spend as they choose. They're looking for entries in the following award categories:

Better environment star
Outstanding achievement star
Healthy community star
Newcomer star
Stronger community star
Young rising star

To nominate an individual or a group, simply do one of the following:

Complete a nomination form by hand (forms can be sent to you by calling 01204 335779 or pick one up at a UCAN centre or Neighbourhood Housing Office). Return details are on the form.

Go online to www.facebook.com/fbboltonathome and upload images and/or video directly onto the Stars of the community discussion page along with your nomination (no more than 500 words).
