People in Bolton are being asked to complete an NHS Survey which has been sent out to over 28,000 residents and could win a £50 voucher.
The primary care trust wants to find out about careas of health and well being in order to see how lifestyle factors affect things and how things may change over time.
The survey will be looking at areas such as ill-health, smoking and diet and will help to shape the future of our health care services.
A random sample of Bolton residents have been taken from GP surgeries.
Anyone who completes the survey will then be entered into a prize draw with the lucky winner receiving shopping voucher for £50.
