If you’ve had a stay in hospital, or even a day visit, it may take a little while before you can get back to normal.

 

Did you know there are organisations that can give you practical support when you get home – or help you prepare before you go in?

 

If you can’t get to or from an appointment under your own steam you may even be able to get help with travel.

Recovering after a hospital stay can feel overwhelming, but Surrey residents may be eligible for support designed to help make the transition home easier.

 

Surrey County Council offers information on:

  • Reablement support
  • Short-term care
  • Home adaptations
  • Equipment
  • Help with daily living tasks
  • Community support services

The aim is to help people recover safely while maintaining as much independence as possible.

This can be especially valuable for:

  • Older adults
  • People with disabilities
  • Those recovering from illness or surgery
  • Family carers needing additional support
