The Live Well Service continues to support the health and well being of the community of Bury during these challenging times.
The Live Well Service offers support on a range of lifestyle related topics which include:
Appointments with the Health Trainers from the Live Well Service are currently held via telephone as staff continue to work from home, support is also available via email and other communication platforms.
People who are referred into the service for support are contacted by a trained staff member and an assessment of their health and wellbeing needs taken.
We offer information and support on how to identify and then set SMART goals and will refer on or signpost other agencies as need dictates.
Clients are offered 6-8 appointments according to need, and an holistic assessment is taken of the current eating habits, any health issues, physical activity levels, sleep quality and alcohol consumption.
Staff use motivational interviewing techniques and behaviour change models to support patients to make life long changes to their health and wellbeing.
We are able to access and provide electronic sources of information on a whole range of subjects.
All topic areas can be accessed by either the online referral page https://www.bury.gov.uk/index.aspx?articleid=14884, clients can self-refer or be referred in by a Healthcare professional or other party.
Telephone referrals can be made by calling 01612537575 (Option 3) at any time and leaving a voicemail message – a staff member will then call back

The criteria for referral is:
Live, Work or Study in Bury and/or Registered with a Bury GP surgery.
